Safety Reporting

Purpose of SAA Reporting System

The purpose of the SAA reporting system is to improve aviation safety by ensuring relevant safety information relating to civil aviation is reported, collected, stored, protected, exchanged, disseminated and analysed.

Why do I need to report?

You are a valued member of the aviation industry, or you are a user of our facility and as such it is your responsibility to report incidents/accidents and safety concerns for a number of reasons including:

  • To comply with SCAA requirements to report safety occurrences
  • To ensure the responsible person(s) or office within the organization receives the information promptly and any investigation and mitigating action(s) can be achieved without delay.
  • To prevent the recurrence of the reported incident and to assist in the management of safety risks.
  • To ensure safe air travel in and out of Seychelles.

Do I get punished If I report hazards or occurrences implicating myself?

The SAA reporting System promotes open reporting. It is not to attribute blame or liability unless following an investigation such disclosure reveals, beyond any reasonable doubt, an illegal act, gross negligence, or a deliberate or willful disregard of regulations or procedures is proven.

This delivers on the SAA’s Just Culture Policy and gearing our efforts towards a more holistic approach through a proactive, evidence-based, risk and performance-oriented safety system.

1. Operational Safety Report

 The following options are available:

i.Occurrence report:
Report on all mandatory occurrences If you are required to submit occurrence reports in accordance with SCAA regulatory requirement due to your profession (ATCO, Aerodrome personnels etc.) submitting an internal report and publishing it to SCAA fulfils your obligation to do so.

ii.Hazard/Voluntary Report:
Report all other occurrences not captured by the mandatory reporting system or other safety related information which you perceive as an actual potential hazard to aviation safety. This report can be submitted confidentially, through activating the option in the confidentiality field.

iii.OHS Report:
Report on work related health and safety hazards, risks and dangers. The purpose is to reduce the risks of incidents and accidents at the workplace and public spaces at our airports. This report can be submitted confidentially, through activating the option in the confidentiality field.

2.External Report

The following options are available.

i.Hazard Report:
Report all other occurrences not captured by your organisation reporting system or other safety related information which you perceive as an actual potential hazard to aviation safety. This report can be submitted confidentially, through activating the option in the confidentiality field.

ii.OHS Report:
Report on work/ related health and safety hazards, risks and dangers. The purpose is to reduce the risks of incidents and accidents at the workplace and public spaces at our airports. This report can be submitted confidentially, through activating the option in the confidentiality field.

Should I report this occurrence?

If ever unsure if something is reportable, please report anyway, particularly if you feel that others could learn from your report. This could prevent someone else from getting hurt or worse. This will indicate a good reporting culture.

Will I receive a copy of my report?

You will receive a PDF copy of your report by email and have the option to copy your report to others of your choosing.  This email and PDF also act as confirmation that your report has been filed so if they are not received it is important that you contact us by email at safetyoffice@saa.sc

How do I provide follow up actions, new information, attachments or any correction/changes to a report?

Click on either “Submit an Internal Report” or “Submit an External Report” and depending on which portal you used for submitting your initial report, using the full report number and the validation code, you can upload further information and attachments and make any corrections or changes.  You will then receive an email and a PDF copy of the revised report as confirmation that it has been filed. If you are unable to access the report again, additional information, attachments, corrections and changes please contact us by email on safetyoffice@saa.sc and quote the relevant report number and validation code.

What happens next?

All reports are reviewed and classified when submitted. Depending on the classification category, reports classified as medium to high risk are investigated to ensure all hazards are identified, risk assessed, and appropriate actions are taken to reduce safety risks.
